I understand that a $250 deposit is required to confirm this reservation. $100 non-refundable if canceled 72+ in advance. Full deposit forfeited if canceled within 72 hours. This deposit secures my booking date and vehicle.

The deposit is applied toward the total trip cost but is partially non-refundable in the event of cancellation, rescheduling, or no-show.*

I agree that the remaining balance must be paid before or at the time of service. Final trip total will reflect time, distance, and any approved add-ons or stops.*

I agree that The Mom’s Helper LLC is not liable for delays caused by weather, traffic, or circumstances beyond its control.*

I agree to indemnify and hold harmless The Mom’s Helper LLC, its drivers, and employees from all claims arising during the trip.*

I understand that respectful conduct is required by all passengers. No smoking, vaping, or unruly behavior is permitted in vehicles.*

I agree that any damage to the interior caused by my group may result in additional charges. Cleaning fees may apply for spills, gum, or staining.*

I consent to sign this form electronically and agree that my digital signature is legally binding.*
